The most advanced stage of chronic kidney disease, where kidney function is below roughly 10% of normal (assessed by the GFR, or glomerular filtration rate). At this stage, the kidneys can no longer cleanse the blood on their own. Replacement therapy then becomes necessary: dialysis or a kidney transplant.
